If your insurance policy is an HMO, you will need a new referral every year from your Primary Care Physician prior to your appointment. They can fax it to our office at (207) 797-9793.
You are responsible for providing ALL insurance cards to the office. We can only bill the insurances that we have on file, so please provide all active insurance cards at the time of service.
